The Team Administrator will be responsible for receiving, processing and forwarding of referrals at the first point of access. Attention to detail and fast paced person who is able to manage referrals in an efficient manner. The post holder will provide an administrative function to Referral and Assessment Hub,providinga same day response to community referrals for adults of a working age, ensuring they are managed in a transparent way and people get appropriate care and treatment in the right place at the right time.

Ensuring referrals are correctly categorised (urgent, routine).

The Team Administrator will have an overview of the patient pathway with responsibility for ensuringreferrals, reviews and appointments are processed and conducted within the required timeframes.

Referral Processing:Coordinating the flow of referrals from Primary Care processing of all referrals into the service using NHS patient systems, ensuring that all relevant Health & Social Care systems are updated in accordance of that referral.

Ensuring referrals are correctly categorised (urgent, routine).

Patient Support:Providing patients with information about the referral process, and other relevant details.

Communication: Communicating with patients, physicians, and other healthcare providers to ensure clear information flow.

Process all referrals received into services, ensuring updates to clinical information systems align with Trust and service guidelines.

Monitor shared administration email inboxes and action in a timely manner.

Coordinating and directing service user contact via telephone call, email, or letter.

Record Keeping: Maintaining accurate and up-to-date patient records, including referral information.

Ensure an effective and efficient booking service is provided with service user negotiation in accordance with the targets stated in theNHS Plan.

Ordering and management of the team stationery, assuring stock levels remain appropriate.
