Job Purpose

To provide person-centred care and support to individuals, promoting independence, dignity, and quality of life. The Support Worker will assist service users with daily living activities, social inclusion, and personal development in line with individual care plans.

Key Responsibilities

Personal Care and Support

Assist service users with personal care (washing, dressing, toileting, grooming).

Support with mobility, including the use of equipment where necessary.

Administer medication in line with policies and training.

Monitor and record service users’ health, wellbeing, and progress.

Daily Living and Independence

Support with meals, shopping, household tasks, and budgeting.

Encourage and enable service users to develop skills and independence.

Assist in accessing community activities, education, and employment opportunities.

Emotional and Social Support

Build positive, trusting relationships with service users.

Provide companionship, encouragement, and emotional reassurance.

Support service users to maintain relationships with family, friends, and the community.

Safeguarding and Compliance

Follow safeguarding policies to protect service users from harm.

Maintain accurate records, including daily notes, care plans, and incident reports.

Work within CQC standards and organisational policies.

Report any concerns to the line manager promptly.

Teamwork and Development

Work collaboratively with colleagues, families, and professionals.

Attend training, supervision, and staff meetings.

Contribute to continuous improvement in care quality.

Basic IT and record-keeping skills.

Qualifications & Experience

Experience in a care/support role (desirable but not essential).

NVQ/QCF Level 2 in Health & Social Care (or willingness to work towards it).

Knowledge of safeguarding, health & safety, and person-centred care principles.
