This new model of care for specialised Reablement will ensure a dedicated team of trained and skilled support workers, competent in supporting individuals with specialised needs to make lifelong sustainable changes to achieve their goals.

The Building Independence Team will offer a more efficient and consistent approach to support which will be a significant step forward in providing comprehensive, specialised support to a diverse range of individuals. This approach not only maximises the potential for individuals to achieve reduction in care needs but also ensures consistency and high standards of service delivery.

• Individuals with neurodiversity

• Support to residential/nursing provision where 1:1 support is identified, offering approaches to manage distressed behaviours and falls

• Support for individuals with early-stage dementia to remain in their own homes.

The successful candidates will join a service that prides itself in seeing people achieve their goals and return to independence.

The candidate will work in a community setting citywide within an individual’s own home to support them to achieve independence, longer-term solutions and aspirations whilst maintaining their privacy and dignity.

The post holder will provide and promote choice and control to the individual which will help them transition to maximum independence through a person-centred approach and partnership working.

The post holder will maintain accurate, up to date records and collect data and information to evidence performance around service delivery to ensure achievement of internal and external targets.
