Key Responsibilities:

Manage post hospital pathways including HomeFirst and step-down beds.

Manage and attend system calls and provide regular system updates.

Supervise Social Workers and unqualified staff, providing advice and support to the team.

Undertake Care Act assessments and Mental Capacity Assessments (MCA).

Apply safeguarding knowledge to ensure safe and appropriate decision making.

Work collaboratively with health and social care partners to achieve safe and timely discharges.

Maintain accurate and up-to-date case records in line with legislation and local authority policy.

Essential Requirements:

Degree/Masters in Social Work and SWE registration.

2+ years of post-qualification experience.

Strong knowledge of UK social care legislation and practice including Care Act, MCA, and Safeguarding.

Experience in hospital discharge pathways.

Right to work in UK.
