The Old Age Psychiatrist will provide specialist psychiatric assessment, diagnosis, and management for older adults presenting with memory issues / mental health issues in a primary care and community setting. Working in close collaboration with, Elder Care Facilitators Gnosall memory clinic, GPs, practice nurses, social care, and community mental health teams, the postholder will help deliver integrated, person-centred care that promotes independence, early intervention, and mental wellbeing among older adults. Key Responsibilities Clinical Duties Provide psychiatric assessment and management for older adults referred from primary care, GPs, Relatives / Carers, and other local pathways with conditions including short term memory Loss, depression, anxiety, psychosis, bipolar disorder, and dementia. Offer consultation and joint case management with the Elder Care Facilitator, GPs and community mental health practitioners. Support medication review, complex prescribing, and deprescribing in collaboration with primary care colleagues. Participate in memory assessment services and contribute to dementia diagnosis and post-diagnostic support. Provide crisis intervention and risk management for patients with acute deterioration in mental state. Support advance care planning, capacity assessments, and safeguarding where appropriate.
