Team Leader – Community Planned Care

Sunderland Care and Support is one of the largest providers of social care services within the North East delivering a range of services on behalf of Sunderland City Council. We are seeking to recruit a Team Leader into our Community Planned Care Services who is able to encourage a workplace culture based on respect and dignity for those in need of care and support.

The service is community based and we provide care and support to customers in their own homes across the city, develop a wide range of skills and maintain their independence within the city of Sunderland. Customers within our Planned Care service have a diverse range of care needs and require a wide range of support.

The Role
We are looking for experienced individuals who can lead and motivate a team of support workers to build a sense of community, encouraging our customers to have a voice, share their views, have choice, and feel at home. Qualified to at least NVQ/QCF Level 3 in adult social care or equivalent, with demonstrable experience of caring for and supporting adults. You will also have experience of supervising and leading in a care setting.

Key Responsibilities and Accountabilities
As Team Leader your role will be to provide support to an agile Service Manager. Team Leaders are responsible for the smooth and safe running of the service, including managing and creating the monthly rotas, updating and managing care plans, supporting and supervising support workers, including inducting new team members, as well as overseeing reporting, such as medication records and health and safety checks. Team Leaders are also expected to work jointly with other professionals to ensure customers receive support which is designed to meet their own personal need.

Working with the Care Co-ordinator, the Team Leader will ensure the provision of high quality, person-centred care and support, supporting practical, social and spiritual needs, leading the service delivery in the service in line with Sunderland Care and Support values and standards and the requirements of the Care Quality Commission [CQC].
