Support Worker – Job Description
Position Overview
The Support Worker provides practical, emotional, and social support to individuals who require assistance due to disability, mental health challenges, age-related needs, or other vulnerabilities. The role aims to promote independence, wellbeing, inclusion, and a high quality of life for each person supported.

Key Responsibilities
Provide Person-Centred Support: Assist individuals with daily living activities such as personal care, meal preparation, mobility, and household tasks.
Emotional & Social Support: Build positive relationships, offer encouragement, and support individuals to participate in community activities.
Health & Wellbeing Assistance: Help with medications (where appropriate), monitor wellbeing, and report concerns to supervisors or healthcare professionals.
Promote Independence: Encourage skill development and empower individuals to make informed choices.
Record Keeping: Maintain accurate documentation, progress notes, incident reports, and care plans.
Safeguarding & Safety: Ensure the safety, dignity, and rights of individuals, following safeguarding policies and procedures.
Collaboration: Work effectively with families, carers, healthcare providers, and other professionals to deliver coordinated support.
Transport Assistance: Escort or transport individuals to appointments, activities, or community access.
Household Support: Assist with cleaning, shopping, budgeting, and organizing daily routines.
