Reablement & Bridging Senior Care Officer

Description
We are seeking a dedicated and passionate Reablement & Bridging Senior Care Officer to join our dynamic team. This pivotal role is designed for individuals who are committed to enhancing the wellbeing of individuals through personalized care approaches, ultimately helping clients regain independence and improve their quality of life. As a Senior Care Officer, you will oversee the implementation and delivery of reablement and bridging services, ensuring that the highest standards of care are maintained. You will work closely with multidisciplinary teams, including social workers, healthcare professionals, and support staff to assess client needs, create tailored care plans, and facilitate a seamless transition through the care continuum. Additionally, you will be instrumental in developing and conducting training programs for junior staff, sharing your expertise and fostering a supportive learning environment. Your leadership and communication skills will play a crucial role in motivating your team to deliver exceptional care. Responsibilities
Lead the assessment and development of personalized care plans for clients to support their reablement and bridging needs.
Coordinate and oversee the delivery of care services, ensuring compliance with relevant regulations and quality standards.
Supervise and mentor junior care staff, providing guidance, training, and support to enhance team performance.
Collaborate with healthcare professionals and social workers to create a multidisciplinary approach to client care.
Monitor and evaluate client progress, adjusting care plans as needed to ensure optimal outcomes.
Act as the primary point of contact for client families, addressing concerns and providing updates on care progress.
Maintain accurate records and documentation related to client care and team performance, ensuring confidentiality and compliance with data protection policies.
Requirements
Proven experience in a senior care officer role or similar position within the health and social care sector.
Strong understanding of reablement and bridging care principles, with the ability to apply them effectively.
Excellent communication and interpersonal skills, with a compassionate approach to client care.
Demonstrated leadership skills, with experience in supervising and mentoring staff.
Relevant qualifications in social care, health, or a related field (e.g., NVQ Level 3 or equivalent).
Ability to work collaboratively within a multidisciplinary team and engage effectively with clients and families.
Strong organizational skills and the ability to manage multiple priorities in a fast-paced environment.

Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £16.66
Location: Cardiff, Cardiff County
