Care Coordinator – £13.50 per hour



The home supports older adults with a range of age-related health conditions, providing person-centred care in a thoughtfully designed environment. You’ll coordinate care on shift, oversee medication administration, and act as a link between care staff, residents, families, and external professionals.



Before You Apply



  • A Level 3 qualification in Health and Social Care is required.

  • Recent experience in a senior care or care coordinator role is essential.

  • Applicants must be confident administering medication and leading a shift.

What You’ll Need



  • Level 3 in Health and Social Care (or equivalent)

  • Experience leading shifts in a residential or nursing home setting

  • Competency in medication administration and record keeping

  • Confident using electronic care planning systems

  • Strong understanding of safeguarding and CQC standards



What You’ll Do



  • Coordinate care delivery during your shift, ensuring person-centred practice

  • Administer and record medication in line with policy

  • Update and maintain electronic care plans accurately

  • Liaise with GPs, district nurses and other health professionals

  • Support the induction and supervision of care staff

  • Monitor residents’ wellbeing and respond to changes promptly
