The Crisis Resolution and Home Treatment Teams work with people who are experiencing a psychological, emotional and mental health crisis. We are a short term intervention team who can provide home visits to support peoples psychological, emotional and practical needs. We actively facilitate signposting and support to attend to other agencies and transfer between teams. The postholder will work jointly with our clinical team supporting individuals who are often facing several social/practical/emotional challenges during their period of crisis. As a Peer Support Worker you will support our service users to develop and create a Crisis, Relapse and Contingency Care Plan, provide limited practical and emotional support to service users within their own homes, social inclusion, support to attend appointments and transition through services and their own unique recovery process. The role of peer support worker has been developed specifically for people who have lived experience of mental health problems requiring support from a Crisis Team. The postholder will directly report to the Team Manager. The service is committed to achieving the principals of an enabling environment becoming a positive and rewarding place to work and study, where people thrive, succeed and achieve positive outcomes.
