Key Responsibilities Include:
• Providing personal care support, including assistance with washing, dressing, grooming, toileting, continence care, and maintaining personal hygiene.
• Supporting clients with safe mobility, moving and handling, transfers, and the correct use of equipment such as hoists where required.
• Assisting and prompting clients with medication in line with their care plans, medication policies, and training.
• Preparing meals and drinks, supporting nutrition and hydration, and monitoring any concerns relating to eating and drinking.
• Supporting clients with daily living activities, maintaining a safe home environment, and promoting independence.
• Providing companionship and emotional support, encouraging social interaction and reducing isolation.
• Monitoring clients’ health, wellbeing, and any changes in their needs, reporting concerns promptly to the management team, healthcare professionals, or relevant parties.
• Supporting clients with appointments, community access, and activities where required.
• Maintaining accurate care records, including visit notes, medication records, incident reports, and any changes observed during care visits.
• Following safeguarding procedures to protect clients from abuse, neglect, and harm.
• Working professionally with clients, families, colleagues, healthcare professionals, and other agencies involved in the client’s care.
The overall purpose of the role is to provide safe, compassionate, reliable, and person-centred care that improves clients’ quality of life while enabling them to remain safely within their own homes.
