To provide holistic palliative care in the last 4 weeks of life To provide emotional, physical and psychological support to patients, their families and carers To work as part of the Hospice at Home team providing packages of care and support for patients in their own homes. Colleagues usually work in pairs but there is some lone working required. Day shift times 0745 2000, 0800-1600, 1200-2000. All colleagues are required to work a variety of shifts across 7 days as part of an 8 week rolling rota. Occasional nights may be required but are not a significant part of current advertised role. Night shift times 2115-0800. To work autonomously to provide planned care for individual patients with the support of, and accountability too, the Hospice at Home Registered nurses To role model the values of the hospice at all times by following the St Peters Hospice PCSW Code of Conduct and to contribute to a positive team culture To promote the safety and well-being of patients and their families at the end of life by following Hospice policies and procedures and working as part of a wider multi-disciplinary team.
