About Prime Years Care Agency

Prime Years Care Agency was founded on a simple belief: care should be about connection, not just ticks on a checklist. We support adults across Chesterfield, Derbyshire and Nottinghamshire with everything from companionship to practical care packages — building real relationships while helping people live with confidence and independence. With our unique MatchMade™ approach, and a genuine focus on empathy over paperwork, we're building a team who care as much as we do. If you want a role with purpose, where you'll make a real difference to someone's day, every day, we'd love you on our team.

The Role

As a Home Care Assistant, you'll be a trusted, familiar face in your clients' homes, supporting them with daily activities, encouraging their independence, and helping them feel safe, valued and well cared for. An ideal Home Care Assistant should be reliable and caring, with great communication skills, who can build genuine, trusting relationships with the people they support.

Duties will include:
• Assisting with personal care tasks such as bathing, dressing, and grooming.
• Supporting with mobility and daily routines in accordance with personalised care plans.
• Providing companionship and emotional support to enhance clients’ quality of life.
• Preparing meals and assisting with feeding if necessary.
• Administering / prompting and reminding of medication as prescribed by healthcare professionals.
• Performing light domestic chores including cleaning and laundry to maintain a safe and comfortable environment.
• Driving clients to appointments or social activities as required.
• Maintaining accurate records of care provided and communicating effectively with the care team.
• Always ensuring compliance with health and safety regulations.
