To provide high-quality, patient-centered care by undertaking home visits for housebound or vulnerable patients with acute, sub-acute, and chronic conditions. The role supports GPs and the multidisciplinary team (MDT) in delivering timely assessments, diagnostics, treatment, and referrals, reducing hospital admissions and improving access to care closer to home.
